Addition Loss Form
Use the following form to add additional lost, stolen or damaged
property to a crime report already filed with the Lubbock Police Department. If
the lost, stolen or damaged property will not fit on a one additional loss form
please print as many forms as needed.
Return the completed form to the Lubbock Police
Department or mail the completed form to:

P.O. Box 2000
Lubbock, Texas 79457.
Do not use this form unless
an original report is already on file with the Lubbock Police Department and you
have the case number to that report. If you have any questions, contact
the Police Department at 806-775-2816.
