PERSONAL HISTORY STATEMENT (PHS)
Candidates need to make the necessary arrangements to have the following certified documents available for submittal with their completed Personal History Packet:
Birth Certificate
Naturalization papers (if applicable)
High School Transcript or G.E.D. Certificate
College Transcripts
Notarized copies are NOT acceptable replacements for the above requested certified documents.
The PHS is DUE by August 15, 2008 at 5:00 pm. The PHS may be brought to the Police Academy at 508 Davis Drive (Reese Center) during normal business hours or mailed to PO Box 2000 Lubbock, Texas 79457. The PHS will be accepted at anytime after notification of passing both the written entrance exam and the physical ability test. Any PHS received or post marked after August 15, 2008 will not be accepted and the applicant will be removed from this hiring process.
